Account Reinstatement For an Organization-based Account (Not Individual)

I am a cloud engineer for a small company. We log on via admin.microsoft.com , signing into our organization using our website link, which takes us to the equivalent of https://login.microsoftonline.com/COMPANYNAME.com/. However, the sign-in is blocked because someone tried to sign into the Manager1 login too many times. I get the following error:

According to Microsoft, to resolve this issue, one must go to Account Reinstatement, and fill out the form at the following link: https://www.microsoft.com/en-us/concern/AccountReinstatement

Yet on this form, there is no way to specify what organization/company the account is beholden to. Regardless, we filled out the form, specifying Manager1 in the account username/ID bar. In the notes, we specified the company name, but to no avail. We received this email:

We know that this account exists under the organization, but the Account Reinstatement form has no place for us to specify what organization under which to reinstate the account! It appears to only be designed for individual users.

How might we reinstate an organization-specific account? Is there a separate form we can fill out?
