About your second question, I'd like to ask if you can clarify that you "didn't have the environment ready." Were you attempting to do an in-place upgrade or clean installation? Or it was a build offered in Windows Updates? Is there any specific error code? While waiting for the answer, here are some suggestions:
- Try a different USB port if you're using bootable installation media
- Check Secure Boot and TPM options in UEFI/BIOS
- Recreate your installation media on another USB
