I found the following deployment example topologies on a PowerPoint slide deck from one of our technical solution professionals. 

Monitoring and Analytics - 5 servers:

1) MOSS server with Web parts and dashboard viewer

2) PerformancePoint Server Monitoring server

3) ProClarity server (32 bits)

4) Microsoft SQL Server

5) SQL Server Analysis Services server

Notes: It is recommended to setup SQL Server and Analysis Services on two different machines.

 

Planning Deployment - 3 servers:

1) PerformancePoint Planning Server (with a Planning process service, Web Services, and IIS)

2) SQL Server

3) SQL Server Analysis Services server

 

PPS Planning + M&A deployment Sample with 200 users M&A part with security by user

1) 5 servers (3 PerformancePoint Server licenses)

2) PerformancePoint Planning server

3) PerformancePoint Monitoring Server and ProClarity Server
