SharePoint 2013: Migrate from SharePoint Foundation 2013 to SharePoint Server 2013

1. Install a clean/new instance of SharePoint Server 2013.
2. Migrate content to new instance and then migrate the content over, e.g. with content database attached, 3rd party migration tool, or site collection export\import.

Apparently (see comment below), installing SharePoint Server 2013 on top of an existing SharePoint Foundation 2013 farm is not supported.
