SCSM 2012 Incident Resolution Satisfaction Surveys on SharePoint 2010

Travis Wright wrote a nice blog on how to create a satisfaction survey on SCSM 2010 and Sharepoint Portal Server 2007.

The steps on SCSM worked but you have to download the SharePoint Designer 2010 and Designer Service Pack 1

After some try and error I figured where to imput the JavaScript to hide the Incident and Analyst that is different on SharePoint 2010 page:

    Hi Carlos, thanks for the blog, I am having problems getting this to work though, my Analyst and IncidentID fields are hidden just fine, but when I view a response, they come up as undefined. Where might I be going wrong?
